How can I obtain a Good Faith Estimate (GFE)?

Summit Radiology is able to provide a good faith estimate via our billing company, Zotec.  Click here to be directed to the Good Faith Estimate form.

How are Auto accident claims handled?

Summit Radiology is not contracted with Auto Insurance Companies.

Summit Radiology will file a claim one time with the responsible Auto Insurance Company when the patient has provided us with that information. We do not hold accounts waiting for settlements between the Auto insurance company and the patient.

Summit Radiology is willing to file a claim with your medical insurance provider. Your Auto insurance company can reimburse your medical insurance for any claims paid that were related to the automobile accident.

How can I reach a billing representative?

You may contact a billing representative from 8:30am to 4:45pm EST Monday – Friday at 260.432.1568 or toll-free at 800.619.6732. If you leave a message, we will return your call as soon as possible.

How do I update my insurance or other billing information?

Call Summit Radiology right away if your insurance information has changed or was never added. Summit Radiology will update the account information and will bill your insurance company. Please have your account number and insurance information ready when you call. A billing representative can be reached Monday – Friday from 8:30am to 4:45pm EST at 260.432.1568 or toll-free at 800.619.6732.

What does “adjustment” mean?

“Adjustment” refers to the portion of your bill that has been adjusted per the terms of Summit Radiology’s contract with your insurance company. 

Why did I receive a bill for a doctor I did not see?

Certain physicians help with your medical care even though you may not meet them, radiologist being the most common. Our radiologist is contracted through the site/hospital where your imaging services were performed.

Does Summit Radiology offer financial assistance?

You may qualify for financial assistance if you meet specified criteria. Learn more about our financial assistance policy by calling 260.432.1568. Please choose option 1 then option 2.

What methods of payment do we accept?

We accept Discover, Mastercard, Visa, cash, and personal check.

How can I pay my bill?

You can pay your bill through our website on our Pay a Bill page or by calling a billing representative from 8:30am to 4:45pm EST Monday – Friday at 260.432.1568 or toll-free at 800.619.6732.

Why didn’t my insurance pay my bill?

Summit Radiology will file claims with your medical insurance provider. Deductibles and/or co-pays, benefit limitations, Insurance program exclusions are examples of fees that are not paid by insurance companies and are the responsiblity of the patient.  Please contact your insurance provider for questions regarding your plan provisions. 

Does Summit Radiology offer payment plans?

A payment plan is available if the balance is greater than $100.00. To set up a payment plan, please call (260) 432-1568 or toll free 1-800-619-6732 option 1 then option 2 for collection department.

Do accounts get turned to collections if left unpaid?

Summit Radiology will turn any unpaid balances to a collection agency after three statements have been sent to the address that was provided by the facility at the time your services were provided. Summit Radiology will also attempt to contact you with the phone number provided by the facility before turning any unpaid balances into a collection agency.
